The Physical Therapist will evaluate and render all services and treatment techniques offered by the Home Care Department. This position will perform initial evaluations of patients in their homes, and also develop appropriate treatment plans including short-term and long-term goals. The Physical Therapist will have responsibilities in reevaluating patients according to departmental policies and procedures, and adjust treatment plans accordingly. Assures timely and appropriate care planning and adherence to policies and procedures by all therapists.
This position will be working within a multidisciplinary team to provide patient care.

The position is located in a Rural Health Clinic associated with a community hospital that is a part of a large health system. The community hospital is a 160 bed Acute Care hospital with a Skilled Care Nursing Facility attached.
The Physical Therapist will participate in community and professional efforts concerned with improving quality of Home health care

Graduate of Physical Therapy curriculum approved by the American Physical Therapy Association, and be eligible to practice physical therapy in the state of Pennsylvania.
Registered and licensed as a Physical Therapist in PA.
One year experience in a health care setting as a Physical Therapist.
Experience with geriatrics and chronic illness
