    Medication Safety

Wednesday, December 10, 2008, 12:44 PM EST [General]

Our CNO shared with us these good points about medication safety.

All healthcare professionals have been taught the five rights of medication administration, these include:

    • Right patient
    • Right dose
    • Right route
    • Right drug
    • Right time

As a healthcare professional there are additional steps that can be taken to ensure patient safety:

    • Check that the order is compete and clearly written.
    • At the station there should be a current nursing drug reference book.
    • If you have questions about a medication call the pharmacy and ask for clarification. Do not administer a drug unless you have received enough information to do so safely.
    • If you see a situation that could result in a medication error, speak up.
    • Be aware of medications that can not be crushed, these include enteric coated and slow release medications. For example, extended release morphine capsules contains both immediate release and sustained release morphine beads. Crushing or chewing the capsule could rapidly release a fatal dose.
    • Be familiar with the list of error prone abbreviations and do not use them in your documentation. [A list can be found on the website of the Institute for Safe Medication Practice.]

A nurse can administer up to 50 medications a day, these additional steps, may seem like an inconvenience but they can prevent a medication error and improve patient safety.
